I believe you have overlooked the earlier versions of MH-E that have
existed in the Emacs distribution for a very long time. [I personally
started using mh-e (MH-E version 3.x) with Emacs 18.x (where x was
somewhere around 34) circa 1988.]
The oldest tarball I happen to have on hand right now is for Emacs
19.34b, so let me see...

The version in emacs-19.34b was MH-E Version 5.0.2 which worked with
both MH version 6.8 or nmh.
