Using the org-mode that ships with Emacs 24.3.50.1 [1] I processed the file using `org-export-to-html` and could not reproduce the issue.

The org-mode from HEAD doesn't have `org-export-to-html` since the backends were re-written and I have to use `org-html-export-to-html` and am seeing this issue.
