Junk is probably one such folder on your outlook.com account. This is created by the server. 2. Those messages which are marked by outlook.com server as spam are going into Junk before reaching Gnus. Hence the split is /not/ working for them. 3. Those messages which escape the outlook spam filter are going through your split chain and ending up in mail.misc.
