I put this on an overlay's before-string property
((margin left-margin)
"1 php-mode")
but I see nothing in the left margin (with left-margin-width set to 20).
Is this a bug or do I misunderstand something? Doesn't it work for overlays?

try e.g.
(overlay-put ov 'before-string
(propertize " " 'display
'((margin left-margin) "1 php-mode")))
i.e. a margin display property on string that is the before-string
you're putting on the overlay...
