Why is etags a C program, and not an Emacs Lisp function? Would it be
desirable to have an Emacs Lisp version of etags?

Maybe. Etags is very fast. It was writen a long time ago. Maybe
nowadays it would make sense to convert it to elisp.

etags is a C program because 25 years ago it would not have run fast
enough otherwise.
