How does one insert/delete (as opposed to change) a byte in hexl-mode?

If a feature of hexl-mode is that insertion/deletion of bytes is
impossible, then what is the intended/recommended method for emacs
users to edit (as in all of insert/delete/change -- not just change)
bytes in a (binary) file?
