In order for an old cache entry to be "wrong", the X server will have to allocate a colormap entry once, then dealloc it, then allocate another color in that space. Do most PCs nowadays have color maps, or do they store the actual color values in the pixel? In another context recently someone said it was the latter. If the latter, we could turn off the caching code on the occasional displays that really have a color map.
