When showing the RefTeX table of contents, I can select a heading to
jump to.
Unfortunately, I don't have a mouse-2 button, and I'm used to jumping
to links with mouse-1.
I have set mouse-1-follows-link to t. Shouldn't the RefTeX table
respect that?
Is there a customization that allows me to set something like the
above option in RefTeX?

Try applying this patch, and setting `reftex-highlight-selection' to
`both'.
