Start thinking what configuration and/or extension
might come into play.
It would be a natural thing to begin with your
initialization for Org-mode, but... should that really
matter to `query-replace'?

What did you query, and replace? And for what buffer,
what modes, etc.? Can you reproduce it by doing the
exact same thing?
