I can't reproduce this here, neither by using kmacro-start-macro nor
kmacro-start-macro-or-insert-counter.  

I tried to add the "TODO" sequence manually or by completing "T", but
both were correctly replicated by calling C-x e.

Maybe you should be more explicit about the context ?
