Even the 2018 edition of POSIX does not require support for int64_t. As I understand it, this was a concession to microcontrollers and Unisys mainframes that are unlikely targets for Emacs (and are on their way out anyway); still, it doesn't hurt to follow the standard when it's easy to do so, as is the case here. Admittedly the portability change was low-priority, but it made the source code smaller and simpler and it used static rather than dynamic checking for some features, so I don't see how it made Emacs "worse".
