They don't need to. They can rely on non-semantic metadata that the origin web 
server provides, e.g., an entity tag. However, if the origin web server is 
misbehaving when a blocking bug is fixed, or if the browser or intermediate 
cache is misbehaving in that particular case, one could see the symptoms you're 
observing.
