True, but that's always been a bug, even before the recent changes.
substitute-command-keys has never been idempotent.

If I understand you correctly, curved quotes will normally be kept in
destination strings so it should be OK. The only exceptions will be if users
customize help-quote-translation to say that they prefer straight quotes, or if
they run in an obsolescent environment that can't display curved quotes.
Neither case should be common.
