I used to have this set to file, but then I _always_ had to add the file
name even for local headlines. I now switched to ido using:
(setq ido-everywhere t)
(setq ido-enable-flex-matching t)
(setq ido-max-directory-size 100000)
(ido-mode (quote both))
(setq org-refile-use-outline-path nil)
(setq org-completion-use-ido t)
(setq org-outline-path-complete-in-steps nil)
(setq org-refile-targets '((org-agenda-files :maxlevel . 5)

which works very well for me, especially once I added the
verify-refile-target.
