copy/paste doesn't work on X11 through the primary selection.

Shrug. I then strongly recommend the approach taken is to create the
x-select-active-regions-enable-primary and
x-select-active-regions-enable-clipboard variable I just proposed
elsepost, and set ..-enable-clipboard to t on msdos, rather than
mysteriously conflating primary and clipboard within
x-set-selection/x-get-selection/x-selection-owner-p
for no good reason.

w32 doesn't change the meaning of passing 'PRIMARY vs. 'CLIPBOARD to
x-set-selection/x-get-selection/x-selection-owner-p , it
just sets x-select-enable-clipboard to t so that killing and yanking
uses the clipboard.
Similarly, it could set the proposed
x-select-active-regions-enable-clipboard
to t by default if you wanted select-active-regions to affect the
clipboard by default.
