If you don't document it then users won't know how
to use it correctly from Lisp (at least not without
studying the code).
That's misleading enough to make me wonder whether,
in that case, it might be better to say nothing at
all about the call-from-Lisp case, forcing Lisp
users to consult the code.
My preference is (obviously) to have the doc tell
the whole story: accurate & complete.
