Did you try to just customize the variable tool-bar-mode? Maybe (I am not sure) the customization is loaded before the frame actually display any toolbar. By adding such line '(tool-bar-mode nil nil (tool-bar)) what do you get? It is a quoted expression... or do you mean that you changed by hand a customization?
