I am using US keyboard, and I could never invoke C-M-%, it simply
remains without function.
On which keyboard is that supposed to work?
Who actually uses keyboard to invoke replace-regexp?

For me, it works just fine. Are you sure you're hitting shift together with
control and alt? On a US keyboard, you need shift to get to `%`. (`C-M-5`
gives you the digit argument, which may seem like nothing, depending on
what you type next.)

I do, whenever I need it. :-) It's not a very convenient binding, given
that you need to press three modifiers at once, but I don't need it that
often, so it's fine.

It works for me in a GUI frame, but not in a tty frame.

I am very sure in it.
But I now see what is the problem, there is some contradiction with IceWM Window Manager. Of course that is why it never worked.
So the issue is closed.

I see you've found the origin of your problem, but just for
completeness's sake:
- You can use `ESC C-%`.
- I never use `C-M-%` and instead use `C-u C-s .... M-%`
(or `C-s ... M-r ... M-%`)

Virtually all of Emacs's "core" keybindings were introduced for/by
people using US keyboards.
