how to create a content menu item, such as "open with emacs..."
when i right click the file or directory?

I think you mean a context menu.  See dired+.el for examples -
http://www.emacswiki.org/cgi-bin/wiki/dired%2b.el.

Look at the code for the mouse-3 context menu: `diredp-mouse-3-menu'.
