Hah! :) It is _usually_ updated by a few poor souls with little-to-no familiarity with the code in question, who have to struggle to figure it out. It is _supposed_ to be updated by the people who wrote the code, (since they know it best), who should feel free to say (if necessary) "I'm not good at documentation, please help me polish this."
