I pushed the following into the Emacs trunk to remove
some more K&R cruft from the source.  The Windows ports
should still build as far as I can see; they
may benefit from some minor cleanup since they no longer
need to worry about the removed macros POINTER_TYPE, PROTOTYPES,
__PROTOTYPES, const, and volatile.
