It looks to me like `c-special-indent-hook' is only run in
`c-indent-line' when `c-syntactic-indentation' is non-nil.
Is this intentional? Should not the doc string for the hook mention that?

This is indeed the case. The CC Mode manual states this on page "Other
Indentation".
