Aren't you going to catch
(query-replace "" "" nil nil nil) etc., i.e. query-replace-regexp
preferably when the user enters the first empty field.

It's legal, and even (occasionally) useful, to query-replace the empty
string with a nonempty string.
I can't really think of a reason why you would ever want to
query-replace a string with the same string, but I'm not sure I see a
reason to generate an error, either.
