In the Customize Face buffer most users will think the "Apply and Save"
button will save their changes to a file, only to unknowingly lose them
upon exit.
Only if they read (info "(emacs) Saving Customizations") will they learn
of the hidden "Save for Future Sessions" button.
I propose the latter button be placed on the interface right beside the
former button, visible from the start.

In what version of Emacs is that, and where do you see "Apply and
Save"? I only see "Apply".

OK, but then "Apply and Save" does save the customizations in a
file--in my init file. It even says so in the echo area. So I think
I'm going to say that I cannot reproduce your problem, neither in
Emacs 25.2 nor in Emacs 26.1.

It saves them to your .emacs ? Maybe it didn't detect you had a
.emacs-custom.el . Anyway, then why is the difference between the
"Apply and Save" and "Save for Future Sessions" button? OK I suppose the
latter would just save and not apply.
(However I have not looked at the code.)

Why is this detail important? Your original report was that it
doesn't save at all ("will be lost upon exit"). Do we now agree that
the customization _is_ saved when you press that button? And that
there's no bug here?

What "Save for Future Sessions" button? I only see "Save for Future
Sessions" in the "State" menu, I don't see a button with that label.

Nothing got saved for me

Ah, so there are two different "states" for saving...
OK I'll close this bug. Customization has always been a disaster for me.
